Complete the Request form below you need to apply with your KKU mail (xx@kku.ac.th or xx@kkumail.com ) ONLY  and open WITH  CHROME or Firefox NOT internet explorer (IE)

the system will sent your request to your advisor then to VDL staff , Finally head of Laboratory in order

if your request is complete you will receive copy of the result

please make an appointment to Laboratory staffs to instruct you the lab rule and how to use the equipment you request before you using it

ฟอร์มนี้ต้องใช้ KKU-Mail ในการสมัคร โปรด Log-in ก่อน จึงจะสามารถเข้าถึงสิทธิ์ได้ และต้องใช้ Chrome หรือ Firefox ในการเข้าหน้าเว็บด้วย ถ้าใช้ internet explorer จะมีปัญหา 

ระบบจะทำการส่งคำขอไปยังอาจารย์ที่ปรึกษา ตามด้วยบุคลากรห้องปฏิบัติการ และ หัวหน้าห้องปฏิบัติการ ตามลำดับ

หากท่านทำการลงทะเบียนสำเร็จ จะได้รับอีเมลล์ซึ่งแสดงผลการลงทะเบียนสำเร็จโปรดเก็บอีเมลล์ไว้

หลังลงทะเบียน ผู้ที่ลงทะเบียน ขอใช้ห้อง หรือมีอุปกรณ์ที่ไม่เคยใช้ให้ทำการนัด เจ้าหน้าที่ห้องปฏิบัติการเพื่อให้คำแนะนำการใช้งานเครื่องมือที่เกี่ยวข้อง
