Sports Physicals
Above, you will find the current CHSAA required physical and consent forms specific to Vail Christian High School athletic participation. Complete the form packet in its entirety prior to your child’s sports physical appointment and remember to bring the completed forms with you to the appointment. Once completed by the parent(s), student-athlete, and physician please submit the entire packet to the VCHS Sports Medicine Department via Submit Physical and Consent Forms.
Please note, the VCHS Sports Medicine Department is not able to conduct a sports physical as they must be conducted by a MD, DO, NP, or PA. Additionally, it is important to note that a sports physical is different from an “annual wellness” exam, as such, there may be an additional cost not covered by your insurance plan. This physical is a functional assessment to determine if your child is physically capable of participating in athletics and as such is deemed an elective physical by all insurance policies. Physicians will often bundle a sports physical together with an annual wellness exam, but please know they are two different exams and subject to a separate fee.

Sudden Cardiac Arrest (SCA) is a condition in which the heart unexpectedly stops beating. SCA is usually caused by an electrical disturbance in the heart. SCA results in death if not treated within minutes. Be in the know and speak with the VCHS Sports Medicine Department regarding further education on SCA.
