At Virginia Consumer-Directed Services, we are proud to provide high-quality, Medicaid-approved home care services designed to promote independence, safety, and dignity for individuals across the Commonwealth of Virginia.
Our care model is flexible and person-centered, offering both Agency-Directed and Consumer-Directed options contingent upon each client’s needs and preferences.
For families who wish to choose and direct their own caregiver, VACDS offers Service Facilitation under Virginia's Consumer-Directed model.
Our certified Service Facilitators support clients and their families by:
Assisting with initial screenings and Medicaid documentation
Helping select, train, and manage attendants (who may be family members or close friends)
Conducting regular home visits to ensure quality and compliance
Updating care plans and monitoring Medicaid eligibility requirements
Maintaining funding streams for care by obtaining authorizations from DMAS & Managed Care Organizations

Who qualifies?
Medicaid recipients who prefer to self-direct care and can manage their attendant (or have a legally authorized representative to do so).
In this model, VACDS takes full responsibility for coordinating care. We assign qualified caregivers (such as Personal Care Aides) to assist your loved one based on a personalized care plan developed by our Nurse Management Team.
Services include:
Personal Care – Assistance with daily living tasks such as bathing, dressing, toileting, mobility, eating, and hygiene.
Respite Care – Temporary relief for family caregivers while ensuring continuity of care and safety at home.
Who qualifies?
Medicaid recipients enrolled in qualifying DMAS waiver programs (such as CCC Plus or EDCD Waiver) who meet the medical or functional criteria for in-home assistance.
Interested in services for yourself or a loved one?
Here’s how:
Call your local Department of Social Services to request & schedule a Medicaid screening for Personal Care services.
Be prepared to answer any questions they may ask and have the following demographic information for the person in need of a screening on hand:
Date of Birth
Social Security Number
Home Address
Presenting Issues and Diagnoses (what conditions or limitations he/she is currently experiencing)
After you provide the necessary information, the representative will either:
Give you an assessment date immediately, OR
Tell you to wait for their call to schedule the assessment.
⚠️ IMPORTANT: If they say they will call you back, be on standby and answer that call promptly. They will only attempt to call once to schedule the assessment. Missing that call could delay the entire process.
On the day of the assessment, please make sure that the client and his/her authorized representative (if applicable) are present and that all screening questions answered in accordance with his/her worst possible day.
If approved for the service waiver, Choose Virginia Consumer-Directed Services, LLC as your preferred provider.
We’ll take it from there — assisting with all documentation and guiding your family through the enrollment process.
