Help to continue strengthening our parent, educator, and student community by becoming a member of the PTO board. We would love to have new ideas, and fresh faces on the Board for next term. You might even enjoy it.

Please complete this form and return it to the office at least 1 week prior to the birthday. $10
If your child's birthday is on the weekend, we will post it on the Friday before.
Messages will read, "Happy Birthday (student's name)" and also receive a treat.
Pay with cash, venmo, or check made payable to Perkins Elementary PTO. All proceeds will benefit the Perkins PTO.
