on our school marquee!

 Cost: $10

Please complete this form and return it to the office at least one week prior to the birthday.

Requests submitted less than one week in advance will cost $15.

The messages will read: Happy Birthday (student's name) and the student will also receive a treat.

If your child's birthday is on the weekend, we will post it on the Friday before.

Summer Birthdays will be posted on the actual birthdate, and the student will not receive a treat. 

After submitting, please email or call the school to confirm that we have received the birthday request. 

Payment: Pay with cash or venmo.

 All proceeds will benefit the Perkins PTO.
