All fighters need the following for kickboxing or thai.  The Paperwork is ALL online.  Physical must be on state form.  Blood work must look similar to the sample below.  No screenshots of portals for bloodwork.

You must have Physical and Bloodwork UPLOADED BY 4PM 10 DAYS BEFORE YOUR FIGHT.


  (Phys) Annual Physical  Your physical must be done on this form and signed by a DOCTOR (MD or DO).  Schedule your physical as soon as possible.  (Your physical is typically good for 1 year.)  As soon as it is complete take a picture and UPLOAD it or email to uskafightsports@gmail.com

 (Hiv-B-C) Bloodwork:  Schedule your blood work and upload  or email to uskafightsports@gmail.com as soon as possible. Always request a copy of results.  The blood work must be exactly what is listed.   Note: Fighters get the wrong hep b test all the time!  Don’t be that fighter.  It is highly recommended that you use “request a test” to schedule your blood work. (Your blood work is typically good for the year IF it is less than 6 months old when first submitted and licensed.) Link to request a test: https://requestatest.com/combative
