If the child is followed by a UCSF PCP, the UCSF PCP will help the SFUSD provider navigate to the right contact. The name and office number of the UCSF PCP should be known by the family. If not, the SFUSD provider may reach out to the appropriate clinic. The UCSF PCP may be in two practices, with the following general contacts:
UCSF Pediatrics (Mt Zion): Send e-mail to UCSF Primary Care Pediatrics Social Work pool: PediatricPrimaryCareSW@ucsf.edu
UCSF Family Medicine (Lakeshore, Laurel Village, China Basin, or San Mateo): contact the practice (main clinic number), or e-mail with social workers that support these clinics- Jennifer Naranjo Morrish at Jennifer.Morrish@ucsf.edu and Alexandra Sovin at Alexandra.Sovin@ucsf.edu.
If the child is NOT followed by UCSF PCP, SFUSD provider will need to find out from parent who the child’s PCP is and start over. Case is closed from a SFUSD-UCSF communication standpoint, as this means the child is not cared for at UCSF.
