PCP: Primary Care Provider listed on child’s school form.  

* For OT, PT, and SLP, UCSF generally does not provide ongoing therapy.  For this reason, the PCP would be best equipped to identify the appropriate contact

** ROI is not required for sharing of information between two healthcare providers for patient treatment purposes.  If the information will be used to inform an educational plan (IFSP, IEP, or 504), or if a non-healthcare provider (teacher or administrator) is engaged, then an ROI is required
