1. Download and fill up the required information and sign the form.

2. Coordinate with the Chapter Treasurer to settle for any pending Chapter Dues. 

The Chapter Treasurer will give you clearance and to be approved by the Chapter President.

3. Submit form to the New Chapter for approval.

4. Submit approved transfer form to the UAP Membership Department via e-mail (membership@unitrdarchitects.ph) to update in the UAP Portal.
