Steps for uploading and the status of your Physical/Medical History Form!

Please use the following link for the UIL Physical and Medical History Form -

1. Log on to Rank One - RankOne Click Here

2. Click on “Proceed to Online Forms”

3. Click “Continue as a guest”

4. Click on “CFBISD Emergency Form”

• Fill out student name, ID number, and click on RL Turner for school attendance. (Must use the LEGAL name that the school has on file)

•  Fill out an athletic emergency form (as completely as possible)

•  Parent and Student must both sign 

•  Click “submit” at the bottom of the page.

5. Click on “Medical History” (EVERYONE must have a NEW Medical History Form)

• Fill out student name, ID number, and click on RL Turner for school attendance.(Must use the LEGAL name that the school has on file)

• Fill out Medical History

•  Parent and Student must both sign

• Click “submit” at the bottom of the page.

6. Click on “Download and Print”

•  Click on Physical (if needed)

• Print 

• Fill out medical history and both Parent and Student sign at the bottom

• Take filled out medical history and blank physical exam form to your doctor to fill out and sign

• Make a copy for your records

7. Email completed forms with both parent and student signatures to RLT Head Athletic Trainer, Paul Anderson, andersonpau@cfbisd.edu.
