A. Birth certificate must be presented. In its absence, a Baptismal or Marriage certificate, BUT NOT AN AFFIDAVIT OR CERTIFICATION.
B. Must appear personally to the Manager during office hours to show himself that his physical condition is healthy and can walk alone;
C. Shall pay all his arrears in his mortuary contributions; Shall make a savings deposit in the amount that is required under the age category he belongs to.
D. EVEN IF still alive, but lapsed member is already sick, can no longer walk by himself, and cannot personally appear to the Manager during office hours, REINSTATEMENT/RE-ACTIVATION of membership to the Mortuary Program is no longer accepted. Should such lapsed member die, NO mortuary aid will be given to him.
When an active member to the Mortuary Program dies, What to do to Claim the Aid-
A.Designated beneficiary/ies shall notify the office by submitting the member’s Death certificate issued by the Civil Registrar.
B.Said beneficiary/ies shall receive the aid LESS indebtedness/liabilities of the deceased, if any. As of Dec 2020 the beneficiary will receive P 43,000.00
An advance amount of aid is usually given after determining the indebtedness/liability of the deceased, while the remaining aid shall be claimed as advised several days to give the management ample time to compute exact amount due.
If the deceased is a co-maker to a borrower, the other deposits may only be withdrawn by the beneficiaries after the BORROWER shall have fully paid his loan.
