Requirements

Thank you for your interest in becoming a part of our family here at the Senior Citizens Centre! Before a decision is made regarding admission of you, or your loved one, the following documents are required: 

• Completed Application Form (attached below)

• Copy of senior's Identification Card, Passport or Driver's Permit

• Copy of Next of Kin's Identification Card, Passport or Driver's Permit

• Phone Number and Address of Next of Kin

• Full Medical (attached below)

• Source of funding (e.g. pension slip)
