Application -

Annual Permission Form and Consent to Treat - This form is required on an annual basis. It give permission for your scout to attend BSA Scouts activities as well as for leaders to get treatment for your child.

Annual Health and Medical Record - Parts A & B are required for all events 72 hours or less in duration. Parts A, B & C are required for events greater than 72 hours in duration (Summer Camp, Backpacking Trips, etc).

Payment Options (Although Check is preferred):

Check- Payments can be dropped at Michael Walton’s home and made payable to: SJSCS

Please include the scout/scouter’s name in the Memo line.

PayPal - Payments made via PayPal must be arranged with Michael Walton (processing fees need to be calculated).
