Medical Forms

All participants in all Scouting activities MUST complete Part A, B and C of the Annual Health and Medical Record

Part A is an informed consent, release agreement, and authorization that needs to be signed by every participant (or a parent and/or legal guardian for all youth under 18). 

Part B is general information and a health history.  Please make sure leaders are aware of serious medical conditions (severe allergies, important medications, limitations on physical activity, etc)

Part C is required for specific activities.  This form must be filled out by a licensed health practitioner.
