STEP ONE: Please fill out the Director/School form.
STEP TWO: Please fill out one student nomination form for each student. Begin with your highest-ranked student and proceed in order.
The student nomination form has space for one student, but it can be completed multiple times. Please complete the form, submit it, and then repeat it for each student you would like to nominate. We encourage you to nominate as many students as you like; however, please keep them in rank order by starting with your best student and then submitting student names in descending order. Also, we pay close attention to the director's comments. Your comments are being used to break "ties" between two seemingly equally qualified students. If you do not include a comment for a student, they may not be selected for the ensemble. We are always short trombones, double reeds, and French horns. Therefore, you may have more students selected for the ensemble if you nominate one of these instruments.
We will take at least two nominated students from every school. Initial placements will be made in the order that we receive the nominations.
Please look at the rating scale carefully. The number "1" is the best possible rating.
