Associated Factors that Contribute to Readiness for Healthcare Transition 

Study invite email to eligible patients:

Hello, you are being invited to participate in a research study investigating what influences readiness to transition to adult care. This will be a one-time online survey that will take approximately 20-45 minutes to complete. You will be asked a series of questions to determine what factors influence transition readiness.

To be eligible you must be a young adult between the ages of 18-25, be an English speaker, and able to answer survey questions independently. You must also be diagnosed with a chronic condition, examples include but are not limited to, Inflammatory bowel disease (IBD), Congenital Heart Condition or other heart condition, Sickle Cell Disease, Diabetes, or other Endocrine Disorder, Muscular Dystrophy, Cystic Fibrosis, Epilepsy, Asthma etc.

To thank you for your time in completing this survey, you will be entered into a drawing for a chance to win one of two $100.00 electronic gift cards at the end of this survey.
