STATEMENT OF HOMES PURPOSE
Somerset Residence has been established to provide specialized services 24/7 for up to (5) mentally ill and/or developmentally disabled male adults. The program and services provided at the Somerset Residence shall include necessary basic self care and rehabilitation training for the residents. Certified and trained caregivers will teach and reinforce skills in basic self-care, dressing, grooming, eating, bathing, toiletry and following simple directions.
Personal adjustment services will be provided to the residents through counseling and individual therapy. Somerset Residence will provide health services as needed or will assist in establishing the necessary contractual agreements.
A PERSON SHALL NOT BE DENIED ADMISSION OR DISCHARGE BECAUSE OF RACE, RELIGION, COLOR, NATIONAL ORIGIN, AGE, SEX, MARITAL STATUS, PHYSICAL OR MENTAL HANDICAP.
Somerset Residence shall not admit or keep any prospective resident who requires continuous nursing care, whose behavior requires isolation or restraint, nor admit a prospective resident whose requirements and service needs are incompatible with those of the other residents in the home. Residents without guardians have the right to select their own living arrangements.
At the time of admission and at least annually, each resident transferring from another home or state facility must have a current written assessment plan out-lining the specific services appropriate to the resident’s needs. The written assessment plan will be completed by the licensee with or without the cooperation of the resident’s guardian or case management staff within 30 days of the placement date. A resident admitted shall have a written health care appraisal completed within a 90 day period before resident’s admission to the home, or, in emergency admissions, shall obtain the appraisal within 30 days of admission. A written health care appraisal shall be completed annually thereafter. At the time of a residents admission a licensee shall complete a written resident care agreement. All potential residents will be involved in a pre-admission interview conducted by administrative staff.
Somerset Residence does not accommodate a bedfast person(s) requiring continuous nursing care. All residents must be ambulatory. All acceptable medical conditions must be controllable with medication or diet.
All types of resident behaviors will be considered for placement in the home. More severe behaviors will be closely monitored in cooperation with the Residential Care Manager (RCM), I-team, psychologist, social worker, and or psychiatrist.
Somerset Residence will not accept residents who have a history of arson, or who aren’t ambulatory.
Resident’s must be capable of engaging in community activities and/or in-house structured activities. All residents must be involved in some type of developmental programming, and/or training activities. No resident shall be out of his/her programming for more than 30 days without a legitimate reason.
Private pay residents may be billed for Title XVIIII (19) funding. Private pay residents are charged for services on an individual basis and must agree in writing for the cost of services.
Somerset Residence accepts adult male residents.
