INTRODUCTION

You are being asked to take part in a research study.  Please read this paper carefully and ask questions about anything that you do not understand.

 

WHO IS DOING THIS RESEARCH STUDY?

The person in charge of this research study is Dr. Stacie Furst-Holloway of the University of Cincinnati (UC) Department Psychology. There may be other people on the research team helping at different times during the study

 

WHAT IS THE PURPOSE OF THIS RESEARCH STUDY?

The purpose of this research study is to examine the characteristics and dynamics of effective top management teams.

 

WHO WILL BE IN THIS RESEARCH STUDY?

About 100 people recruited from the research team’s personal and professional networks will take part in this study.  You may be in this study if you are or have been a member of a top management team.

 

WHAT WILL YOU BE ASKED TO DO IN THIS RESEARCH STUDY AND HOW LONG WILL IT TAKE?

You will be asked to participate in an on-line survey that takes about 10 minutes to complete.  The survey will ask a number of questions that ask about a top management team that you are or have been a part of as well as some questions about you and the organization supported by the top management team.  The link to this survey is included at the end of this information sheet.

 

ARE THERE ANY RISKS TO BEING IN THIS RESEARCH STUDY?

 It is possible that some questions may make you uncomfortable.  You can refuse to answer any questions that you don't want to answer.

 

ARE THERE ANY BENEFITS FROM BEING IN THIS RESEARCH STUDY?

You will probably not get any benefit from taking part in this study.  But, being in this study may help researchers and workforce leaders better understand how to create and sustain effective top management teams. 

 

WHAT WILL YOU GET BECAUSE OF BEING IN THIS RESEARCH STUDY?

You will not be paid to take part in this study. 

 

DO YOU HAVE CHOICES ABOUT TAKING PART IN THIS RESEARCH STUDY?

If you do not want to take part in this research study you may simply not participate.

 

HOW WILL YOUR RESEARCH INFORMATION BE KEPT CONFIDENTIAL?

 We will keep your information private by:

 

·       assigning your survey with a Study ID code rather than asking for your name;

·       limiting access to research data to the research team;

·       keeping research data on a secured drive that is accessible only by members of the research team

 

Your information will be kept in a password-protect file until the study is completed. After that, we will delete any personal information that could link your identity to your responses.

 

The researcher cannot promise that information sent by the internet or email will be private. However, we will take several steps to make sure your identity and study responses are not disclosed beyond the research team.  Specifically:

 

·       UC recommends that raw data be kept for a minimum of two years after the study is closed. After that time, we will delete any identifying information from those files. 

·       The data from this research study may be published; but you will not be identified by name. 

·       Information that could identify you will be removed from the study data. The study data will not be used or shared for future research studies.

 

Agents of the University of Cincinnati may inspect study records for audit or quality assurance purposes.

 

WHAT ARE YOUR LEGAL RIGHTS IN THIS RESEARCH STUDY?

 

Nothing in this consent form waives any legal rights you may have.  This consent form also does not release the investigator, the institution, or its agents from liability for negligence.

No one has to be in this research study.  Refusing to take part will NOT cause any penalty or loss of benefits that you would otherwise have.  You may skip any questions that you don't want to answer.

Also, you may start and then change your mind and stop at any time.  To stop being in the study, you should simply exit the online survey portal.
