Intake Assessment

All consumers will partake in an initial intake assessment.

Trauma Assessment 

A trauma assessment helps gain insight and understanding of how a stressful life experience has caused a disruption in balance, and the inability to manage the day to day without trauma symptoms. It is the best way to identify the existence and the extent of trauma related problems. Using a vast of tools, this will truly be a life changing discovery process. It will be the guide to any re-education that a person could consider when working towards personal growth and other areas of life. 

Assessment of Reunification 

CARE Assessments are great for those families needing a neutral party to provide a written professional assessment of mental status, parenting strengths and needs, and support structure. A CARE Assessment is not a home study. I am not approved to do home studies. The difference is that this assessment will focus on the individual's strengths and needs but will not assess the safety of the home by doing a home inspection, or performing background checks.

This is a factual report and a great point of reference for unique family circumstances. It will be very important for the person being assessed to be open, and honest. I am confident that my clinical expertise, non-judgmental approach and knowledge will help make your encounter be a positive and rewarding one.

Circumstances where a CARE Assessment maybe useful:

This type of assessment typically takes two sessions, and the cost for the initial sessions will be $450 due to the extensive time it will take to provide a thorough interview, review of intake assessment and forms and provide a written report. If additional sessions are needed after the initial, the cost will be $100 per hour.

Before your first appointment, you will need to have the following information available to help with the report:

At the completion of the assessment, a copy will be provided to the individual being assessed. If there are immediate safety concerns, I will have to disclose those safety concerns to the appropriate authorities per my social work code of ethics. If I receive a court order to release information, I will have to release all information requested by law. I am not able to participate in court subpoenas and the report given to the client should be sufficiently informative.
