NOTICE: If you did NOT register your child through the online Sportsengine platform then you will need to complete the waivers through this ONLINE WAIVER FORM and then see your town's player agent for any additional info, 

Carpooling - Participating Adults who are not also acting as a legal guardian, shall not ride in a vehicle alone with an unrelated athlete who is a minor, absent emergency circumstances, and may only drive with at least two other minor athletes or another adult at all times, unless otherwise agreed to in writing by the minor athlete’s parent/legal guardian in advance of each local travel. Carpooling Waiver 

Player Playing up - Per TCYSA policy it is the sole decision of a town's player agent to allow a player to play in a higher age division.  TCYSA follows the guidelines from NYSWYSA and due to insurance reasons a player playing up waiver is required to be carefully read and signed by the parent/guardian of the player.
