4. Nature of the business.

5. No. Employees (men and women)

6. List of relatives actively participating in business.

7. MOA (applicable)

8. Declaration letter of holiday (if holiday given is other than Sunday).

9. Signature declaration letter.

10. Address proof (like Electricity bill or Rent agreement or NOC).
