The following forms are required and must be turned in at evaluations (or prior to team selections):

1. Uniform Agreement

2. Parent Code of Conduct

3. Lindsay's Law

4. Concussion Awareness 

A physical and the below form is required to be scanned and sent to: raphaelphysicals@hotmail.com. 

DO NOT bring the physical to evals.
