Yep, there’s paperwork
Each child must have the proper paperwork. On this website there is a tab that says Required Paperwork. Please print and complete all of the forms on this site. Please label each form in the top right hand corner with your daughter’s first and last name and grade. Your daughter must bring this paperwork to evaluations (exception is the physical form).
All athletes must have on file with St. Raphael CYO a fully completed Pre-participation Form which consists of both a Physical and an Emergency Medical Authorization (EMA),which is found on the website. These forms are valid for 13 months from the date of the physician's exam. Failure to have a current set of forms on file will prevent the athlete from participating in any practice or game. Forms are to be sent to raphaelphysicals@hotmail.com or mailed to
DO NOT BRING THE PHYSICAL FORMS TO EVALUATIONS. They must be submitted as stated above. The forms must be scanned, photos do not work.
Coaching
If you are interested in coaching, please contact me. All coaches must be CYO Certified and complete an application process through St. Raphael. This includes attending both a Virtus Training Session and the Coaches Development Program. Please contact Jen Kennedy (jenken1465@gmail.com) for more information on registering for these classes.
There will be three teams of approximately 8-11 girls. 6 girls play on the court at one time. The girls will rotate in and out to get equal playing time. In grades 3-5, we play for development. Teams will be split evenly. Team, teammate, or coach requests will not be accepted. Girls will learn the game, get to know their teammates better and have fun!
Attendance
I know many of the girls play soccer or other sports. Unfortunately, we can not plan practice times or dates around your child. It is your responsibility to let the coach know if you have a conflict 3 or more days before the game- the sooner the better. If your team has to forfeit a game due to not having enough players, the parents will be charged a fee.
Practices and games
There will be two practices a week held sometime between 4-8:30 at St. Raphael School. Practices will begin the first week of August and run through mid October. Practice times will be based on the coach and gym schedule. Practice days and times will be announced by the coaches. Games are often on Saturdays, however it is not unusual to see a game on Sunday afternoon. There is typically one game a weekend through mid-October. However, last year, some teams did have double headers. The fourth-fifth graders will play in one tournament- during the tournament (usually one, sometimes two weeks/end) you will have more games. Grades 6-8 compete in two tournaments.
Uniforms
The girls will receive a CYO jersey. The girls play in black volleyball shorts. CYO rules require all the girls be uniform, so this means no writing or logos on the black shorts. Knee pads and indoor court shoes are required. (Please note court shoes do not mean volleyball specific shoes. It means shoes appropriately soled for use in a gym.) No jewelry is permitted.
A/B Teams
In grades 6-8, teams are divided into A and B teams. During evaluations, independent evaluators will evalute girls. 8-10 girls will be chosen to be placed on the A team. The A teams will compete in the CYO Elite or A division. Attendance at all practice and games is mandatory for all A team players. An athlete can not miss (which includes come late or leave early) an A team practice or game for another sport or activity. As an A team player, attendance is mandatory. If your daughter if volleyball is not your daughter's priority sport, please remove her from consideration for the A team.
If your daughter misses an A team practice or game, they will sit out a game.
B team attendance is important as well. Please understand that both teams are a commitment. Attendance impacts the entire team. Please consider this if your daughter will have a difficult time making majority of practices or games due to other commitments. If your team has to forfeit a game due to not having enough players, the parents will be charged a fee.

Do not take a picture and try to send that, you must scan it. Questions can be directed to the head of physicals, not the sport commissioner.
