Documentation Process
Initial Evaluation
1) SPEAK OUT! Initial Evaluation
2) SOAP Notes
3) SPEAK OUT! Progress Note on graduation day (8th visit) or last visit of initial treatment session
4) See for 4-6 week follow-up visit then make a copy of your SPEAK OUT! Progress Note you completed on the 8th visit if you're client is ready to be discharged and edit it to say SPEAK OUT! Discharge Summary and edit it accordingly (Change "Progress Summary" under IMPRESSIONS to "Discharge Summary"); otherwise, continue with SOAP notes and make a copy of the previously completed Progress Note and edit it to be a Discharge Summary on your final visit.

GOALS:
Select only the goals that are appropriate for your client. You may also write different goals if you wish.
LONG TERM GOALS: (You do not need to write long-term goals while you are a graduate clinician, but I wanted to provide these examples for you as you will have to write long-term goals when you are in the real world).
The client will use intentional speech to maximize functional communication by achieving 80% accuracy in producing clear, audible speech (minimum 72-78 dB) across three different settings (i.e., home, therapy, and community) as reported by the client and caregivers over three consecutive sessions.
The client will achieve a score of less than 11 on the Voice Handicap Index-10 indicating functional vocal quality (Modify this goal to adjust the numbers so they are achievable for your client).
SHORT TERM GOALS:
Decibel Level Goals:
1) The client will independently demonstrate all six SPEAK OUT!® exercises as indicated by the SPEAK OUT! Therapy Program.

1) Given verbal and visual cues from a caregiver or friend, the client will demonstrate all six SPEAK OUT! exercises as indicated by the SPEAK OUT!® Therapy Program.
*For goals 2-7, you must include a level of independence such as "independently" or "given verbal and visual cues from a caregiver or the clinician." Remember, if you put independently, you can't say anything to help them. Be sure to evaluate the appropriateness of that goal as for most clients it will not be appropriate.
2) The client will achieve a decibel level of 85-90 dB during warm-up exercises across two consecutive sessions as indicated by the SPEAK OUT! Therapy Program.
3) The client will achieve a decibel level of 85-90 dB during "Ah" exercises across two consecutive sessions as indicated by the SPEAK OUT! Therapy Program.
4) The client will achieve a decibel level of 85-90 dB during glide exercises across two consecutive sessions as indicated by the SPEAK OUT! Therapy Program.
5) The client will achieve a decibel level of 80-85 dB during counting exercises across two consecutive sessions as indicated by the SPEAK OUT! Therapy Program.
6) The client will achieve a decibel level of 75-85 dB during reading exercises across two consecutive sessions as indicated by the SPEAK OUT! Therapy Program.
7) The client will achieve a decibel level of 72-78 dB during conversation exercises across two consecutive sessions as indicated by the SPEAK OUT! Therapy Program.
Exercise Completion Goal:
The client will complete SPEAK OUT! exercises 2x/day, reported verbally or recorded in a tracking log, seven days per week to achieve intentional speech during daily routines and activities.
Cueing Goal:
Goal: The client’s caregiver or friend will demonstrate the best cue to elicit "intentional speech" from the client during conversational activities in 90% of opportunities.
Objective 1) Following education from the clinician, the client’s caregiver or friend will voice good understanding of how to cue the client to elicit "intentional speech" during conversational activities in X% of opportunities.
Objective 2) The client’s caregiver or friend will demonstrate an understanding of when to cue the client to elicit "intentional speech" during conversational activities in X% of opportunities
Exercise Goal:
During structured SPEAK OUT! therapy exercises, the patient will demonstrate a minimum rating of X/10 on a 10-point Likert scale, as measured by appropriate vocal quality, intensity, and clarity, in 80% of trials across two consecutive sessions.
Connected Conversation Goal:
During connected conversational speech, the patient will achieve a minimum rating of X/10 on a 10-point Likert scale, as measured by appropriate vocal quality, intensity, and clarity, in 80% of conversational turns across two consecutive therapy sessions.
Exercise Goal:
During structured SPEAK OUT! therapy exercises, the patient will demonstrate a minimum rating of X/5 on a 5-point Likert scale, as measured by appropriate vocal quality, intensity, and clarity, in 80% of trials across two consecutive sessions.
Connected Conversation Goal:
During connected conversational speech, the patient will achieve a minimum rating of X/5 on a 5-point Likert scale, as measured by appropriate vocal quality, intensity, and clarity, in 80% of conversational turns across two consecutive therapy sessions.
