Enrollment Requirements

The following requirements must be submitted to the Registrar's Office before admission:

1. Report Card/Form 137

2. Certification of Moral Conduct

3. Photocopy of Birth Certificate

4. Four pcs. 2x2 Colored Photo (with white background)

Enrollment Procedures

1. Go to the Admission Office and fill out the Enrollment Application Form and present all the requirements for Enrollment

2. NOTE: Foreign pupils only - Proceed to the Foreign Students Office and submit the original and photocopy of SSP, ACR, Passport and or VISA

3. Proceed to St. Therese Building for enrollment evaluation.

4. Proceed to Window 7 (Accounting Office) for payment

5. Proceed to Window 4 (Registrar's Office) for the computerized Official Registration Form

6. Proceed to ID Section
