Admission Requirements

The following requirements must be submitted to the Registrar's Office before admission:

1. Report Card/Form 137

2. Certification of Moral Conduct

3. Photocopy of Birth Certificate

4. Four pcs. 2x2 Colored Photo (with white background)

Enrollment Procedures

1. Go to the Admission Office and fill out the Enrollment Application Form and present all the requirements for Enrollment

NOTE: Foreign students only - Proceed to the Foreign Students Office and submit the original and photocopy of SSP, ACR, Passport and or VISA

1. Proceed to the College Dean/Head Office for evaluation and for approval of section and class schedule

2. For those who have NSTP subjects, proceed to the NSTP Office/ACAP Office

3. Proceed to Window 9 (Accounting Office) for payment

4. Proceed to Window 1 of the Registrar's Office for the computerized Official Registration Form

5. Proceed to Library for registration and issuance of library card

6. Proceed to ID Section
