Please complete the following registration form for 2026:

Activity Forms

• Physical: Every year, an athlete must complete a physical form to practice. The physical form is only necessary if the athlete has NOT competed in a sport this academic year.

• Travel Waiver: In order for an athlete to ride home with a parent or guardian, a travel waiver must be submitted to the head coach of the team. One form is required every time the athlete rides separate from the team. This does not need to be filled out it the athlete is riding on the bus with the team.

Co-Curricular Meeting

• An annual co-curricular meeting is required for any student participating in any sort of co-curricular activity.
