Getting started with SPA-LTC is simple. Once your nursing home is participating, your Champion Team can begin building a palliative approach to care using the SPA-LTC resources, education, and support.
1. Complete the paperwork
Review and sign the Year 4 Organization Information Sheet and Consent Form.
2. Complete your self-assessment
Use the SPA-LTC self-assessment to identify your home’s current strengths and areas where additional support may be helpful.
3. Build your Champion Team
Bring together staff who can help support and champion a palliative approach within your home.
4. Learn, connect, and take action
Access SPA-LTC education, resources, and the Champion Portal, and join our Community of Practice sessions to learn from other nursing home Champions.
You don’t have to do it all at once.
SPA-LTC provides ongoing education, resources, and support to help your team build and strengthen a palliative approach over time.
As a Nursing Home Champion, you have access to education through both iTacit and the SPA-LTC Champion Portal.
