Integrated Client Care Meeting

The Integrated Client Care Meeting was designed to help facilitate the SOME Conway Center’s model of integrated care through structured, client centered multi-disciplinary meetings with the goal of enhancing long-term client outcomes. The integrated business model has now been expanded agency wide.

The client care meeting is a platform given to clients/residents and providers to address concerns, challenges, possible barriers etc…it gives the client the opportunity to sit with all their providers and give input on their plan of care and journey here at SOME. During the meetings, there is a standard agenda to include a review of current services, areas of strengths, challenges and an assessment and coordination for other service needs. The duration of meeting is 1 hour or less. Clients, residents, students and providers can request meetings using this request form.
