Returning Scouts, please provide the following:

Print and complete Part A & B of the form for each Scout.

A new form is required to be filled out every year. Scouts may not be allowed to participate in events or activities if we do not have an updated form on file.

2. Participating Family BSA Health & Medical Forms CLICK HERE to Complete

Print and complete Part A & B of the form for EVERY ADULT or SIBLING who will participate in ANY ACTIVITY this year.

Adults & Siblings WILL NOT BE ALLOWED to camp with us if we do not have this form.

3. Photocopy of Health Insurance for Scouts and Participating Adults & Siblings

4. Pack 62 Information Form CLICK HERE to Complete Online
