Please inform us once you have finished the online form, and send us a photo of your HMO and 1 Valid ID
We will validate and confirm your coverage with your HMO dental provider ( this may take a few hours up to five days depending on the provider)
Once HMO card has been validated, we will be informing you of the available slots (HMO schedule varies per branch)
Reservation fee of P200, which will serve as your Asepsis fee/ Infection control fee, has to be settled first before we give the final schedule.
We will only process your HMO validation and schedule request if you submit complete requirements (online form, HMO card & Valid ID).
Cut off time for HMO processing is 5PM.
Reservation fee of P200 will serve as your ICF/ Asepsis fee and is not covered by your HMO and has to be settled separately.

HMO coverage varies from dental providers and company. However, HMO commonly covers BASIC DENTAL services only (simple extractions, mild cases for oral prophylaxis, simple cases for tooth fillings & consultation with general dentist)
Specialized Dental Services such as : Dental Diagnostics, Pediatric, Surgery, Orthodontics, Prosthodonrtics, Endodontics are not covered.
Additional charges may apply depending on the severity/ difficulty of the case upon thorough examination by the dentist

What is ASEPSIS/ ICF fee for?
Dental work requires our dentists and staff be in close contact with the mouth and saliva which can contain infectious bacteria and virus (Herpes, HIV, COVID, Influenza, monkeypox etc) which can infect our staff and other patients if infection control protocols are not implemented properly.
Please be informed of our Asepsis/ PPE Fee which is P200 as we are required to have a high standard of infection control and have several protocols to maintain the cleanliness of the clinic. This fee also includes the DENR Infectious waste collection charges.
Subsidized infection control fee for additional disinfection, upgrades for infection control/ hazard fee/ PPE worn by the staff, (we change to fresh ones per patient) and fresh PPE you need to wear to protect our staff other patients and yourself.

Will HMO shoulder or reimburse my reservation fee?
If no other charges will be incurred during their dental appointment. the reservation fee will be converted to the asepsis fee/ ppe fee which is not covered by your HMO.
Other charges that I should know about?
Please be reminded that HMOs cover mild cases of cleaning (oral prophylaxis), tooth restoration (pasta) and simple extraction (bunot).
If your case upon consultation cannot be covered by your HMO benefit, (example: deep cleaning or dental surgery) additional charges will have to be settled in the clinic. Our
dentist will be informing you upon consultation regarding your coverage and if there will be additional charges.
For extraction cases, Xrays are mandated for proper diagnosis and treatment planning. Dental Xrays are usually not covered by HMOs.
I have Braces/ lingual retainers/ fixed retainers from another clinic and I want to have my dental procedure using my HMO.
Unfortunately our dentists cannot accommodate you as of the moment. Please schedule your Dental procedure with your attending dentist.
For Dental Surgery referrals, please present your referal prior to booking with us.
Is tooth filling, restorations or veneers for the front teeth included in my HMO?
Unfortunately this is a cosmetic procedure that requires different materials (composite brands and techniques) in which the cost cannot be covered by HMO.
Please book with our Cosmetic dentist for front teeth restorations. You may initially use your HMO for the initial consult for our dentist to be able to do proper referral to our senior cosmetic dentist if your case warrants it.
I WANT TO USE MY HMO FOR MY CHILD UNDER HMO GENERAL DENTIST
Our general dentists can handle the following pediatric cases:
1. 4-13yo for Cleaning & fluoride, as long as the child is cooperative.
2.Dental Sealants, as long as cooperative
3. Mild restoration of PERMANENT TEETH, as long as cooperative
4. Tooth extraction as long as the child is cooperative
Your attending dentist will be the one to decide if treatment can be done or if the case needs referral to other specialists.
Our General Dentists are also not able to handle Children with special needs. (in case of non declaration, your reservation fee will be forfieted)
Please be informed that our Pediatric dentists are not under any HMO.
CAN I JUST BRING MY PPE SO THAT I WILL NOT PAY THE ICF FEE?
Unfortunately we do not allow this anymore since we have to comply with DENR waste segregation. This includes treatment and proper disposal of PPEs. The waste management fees are not covered by your HMO.
