2026 Osteopathic Medical School Graduate: Postgraduate Training License Application

• Part of New Intern Onboarding- ALL incoming MD PGY1s need to apply in the month of May, before June 1, 2026

• Apply for Osteopathic Postgraduate Training License

• Download Checklist from the OMBC website

  1. Complete & submit the BreEZe online application

  2. Pay application fee online through the BreEZe portal. Save Receipt to submit for full reimbursement.

  3. Submit completed form OMB.26 to Miranda.So@hhs.sccgov.org (DO NOT DATE THE FORM- the OMBC will not accept it before your date of graduation, so we will date and submit it for you later in June)

  4. Request for your med school to submit the following directly to the OMBC: (may send in May/ June)

     1. Completed final official transcript

     2. Medical school diploma (you can also submit a copy yourself, OMBC is ok with this) 

  5. Request NBOME to submit official COMLEX Level 1 & 2 exam report to the OMBC

  6. Residency Program Forms: We will complete and submit the Enrollment Form (OMB.23). Nothing you need to do for this. Do not complete yourself.

  7. See "PTL Application Pointers" section below regarding Fingerprinting. DO NOT do on your own.

• PTL Fee Reimbursement (paid to OMBC)- Covered by the County, one time
  To claim reimbursement: Email the following items to Miranda.So@hhs.sccgov.org
  1. Complete & Sign Form: FORM 2025
  2. Receipt (confirmation of purchase w/ date + item + cost)- you must pay with your own credit card/ bank account. No reimbursement if purchased with another person's payment method.

Postgraduate Training License Holder in CA (For PGY2s+)

• Apply for Initial Physician & Surgeon's License

• Must have completed at least 12 months of postgraduate training

  1. Internal Medicine Categorical Residents: apply at the END of PGY3

  2. Preliminary & Transitional Year Intern: seek guidance from your advanced programs

PTL Application pointers for ALL:

ALL: Fingerprinting/ Live Scan Forms: No action needed. We will have a LiveScan service and Notary available at Orientation on 6/18. We will print and these will be available at orientation on 6/18 for you to sign. Do NOT go find a fingerprinting/ notary service separately. Please bring your checkbook or credit card to pay for these services. You can submit these for reimbursement through your ed funds after you start.

Osteopathic Applicants: 

The OMBC requires you to have a middle name in order to use the online application. When asked whether you have a middle name, select “YES” and proceed. If you do not have a middle name, in the Middle Name space on your application, enter “N/A”.
