2026 New Intern Onboarding: Please submit your PTL application to your respective medical boards during the month of May 2026. Read all the instructions here carefully- esp the DO applicants.
California Medical Board and Osteopathic Medical Board will no longer accept paper-based application forms. Applications should be submitted electronically through BreEZe.
New hires enrolled in ACGME programs must obtain their medical license within 180 days of enrollment.
*All clinical activities must cease if medical license is not issued within the period of grace.
Please make sure to put your FULL LEGAL NAME on ALL documents. Consistency is key as discrepancies will result in processing delays.

2026 Osteopathic Medical School Graduate: Postgraduate Training License Application

Apply for Osteopathic Postgraduate Training License
Download Checklist from the OMBC website
Complete & submit the BreEZe online application
Pay application fee online through the BreEZe portal. Save Receipt to submit for full reimbursement.
Submit completed form OMB.26 to Miranda.So@hhs.sccgov.org (DO NOT DATE THE FORM- the OMBC will not accept it before your date of graduation, so we will date and submit it for you later in June)
Request for your med school to submit the following directly to the OMBC: (may send in May/ June)
Completed final official transcript
Medical school diploma (you can also submit a copy yourself, OMBC is ok with this)
Request NBOME to submit official COMLEX Level 1 & 2 exam report to the OMBC
Residency Program Forms: We will complete and submit the Enrollment Form (OMB.23). Nothing you need to do for this. Do not complete yourself.

PTL Fee Reimbursement (paid to OMBC)- Covered by the County, one time

PTL Application pointers for ALL:
ALL: Fingerprinting/ Live Scan Forms: No action needed. We will have a LiveScan service and Notary available at Orientation on 6/18. We will print and these will be available at orientation on 6/18 for you to sign. Do NOT go find a fingerprinting/ notary service separately. Please bring your checkbook or credit card to pay for these services. You can submit these for reimbursement through your ed funds after you start.
