HOW TO REQUEST ANNUAL PHYSICAL EXAM (APE) UNDER INTELLICARE.
The Annual Medical Examination (AME) or Annual Physical Exam (APE) is usually conducted from September to the last quarter of the year. Prior to submission, an official memo will be released to initiate the APE coordination process.
Entitlement: Only regular employees with an active HMO account under Intellicare are eligible for the APE/AME.
To request APE under Intellicare, please follow the steps below:
Consolidate Employee Information
Prepare a list of all employees who will undergo APE. Include the following details
Full Name
Intellicare Account Number
Identify an Accredited Facility
Choose a facility accredited by Intellicare. Note: Not all Intellicare-accredited facilities are authorized to conduct APE, as some may not offer the complete set of required tests under the APE package. Kindly confirm with the facility first.
Fill Out the Intellicare Excel Template
Encode all consolidated employee information into the official Intellicare Excel Template. 📄
Submission for Processing
Email the accomplished template to the corporate health services email:

Wait for the Letter of Admission (LOA)
Intellicare will process the request and issue the LOA.
Processing time may vary depending on the number of employees included.
B. INSURANCE under MANULIFE
Coordination with HR for Manulife Insurance Benefits
FORMS 📄
The Human Resources Department shall ensure that all necessary forms and supporting documents are readily available and accessible to employees who intend to claim benefits under their Manulife insurance coverage.
This includes, but is not limited to:
Medical or health-related forms (e.g., Physician’s Statement, Attending Physician’s Report)
Proof of medical consultation or confinement
Official receipts for medical expenses (if reimbursable)
Diagnostic test results and medical certificates
Death or disability claim forms, when applicable
Required APE documentation, when related to the benefit being claimed
HR must ensure that these documents are:
Up-to-date and in the correct format as prescribed by Manulife
Properly explained to employees on how and when to use them
Available upon request or ideally housed in a shared folder or HR portal for convenience
Timely access to the required forms will expedite the processing of claims and help employees maximize their insurance benefits.
