ONLINE DOLE SUBMISSION
Relative to efficient delivery of programs and services of Department of Labor and Employment – National Capital Region, the Occupational Safety and Health Administrative Reports namely, Company OSH Program, Annual Medical Report, Annual Work Accident / Illness Exposure Data Report, Report of Safety Organization and Minutes of the Meeting of Safety and Health Committee, will now migrate from Google Forms to System-Based Format effective 01 January 2023 through our official website at :
https://clients.ncr.dole.gov.ph.
*Submission to a Non-jurisdiction or outside NCR will depend on the DOLE's Regional office's catchment area. Seek advise from your safety head/officer.
1.) Work Accident/Incident Report (WAIR)
It shall be submitted, WAIR template, for every accident or illness to the Regional Office having jurisdiction on or before the 20th day of the month following the date of the occurrence.
1.1 DOLE-BWC-OHSD-IP-6-WAIR-A; and
2.) Annual Medical Report (AMR)
Consolidation of data must be between the first day of January (or from the month when the project was established, for the new project) UNTIL the end of December, of the previous year. SCIC practice of submission to DOLE is on or before the 20th day of January, but, may also be submitted until the last day of the month of March, present year, as per OSH Standards, 2017.
2.1 DOLE/BWC/OHSD/OH-47 A

3. Annual Work Accident/Illness Exposure Data Report (AEDR)
This report shall be accomplished whether or not there were accident/illness occurrences during the period covered and submitted to the Regional Labor Office or local government having jurisdiction not later than 30th day of the month following the end of each calendar year.
