Safe and effective communication during patient handoffs is a cornerstone of patient safety, particularly in high-acuity environments such as the emergency department (ED). Communication breakdowns remain the leading root cause of sentinel events, contributing to near misses, preventable errors, and patient harm. The SBAR (Situation, Background, Assessment, Recommendation) framework provides a structured, standardized approach to enhance clarity, accuracy, and accountability during transitions of care. This toolkit brings together 14 scholarly and professional resources to support nurses, charge nurses, and educators in implementing SBAR as the standard handoff method in our ED.
