You can simply share this website with him/her, OR click the button yourself and share the URL for the form with him/her. If you are having 2 sponsors, each person must fill out her/his own form.

If you would prefer a printed copy of the form, please contact the parish office. You will be responsible for providing the completed form to the office by Nov 20.
Want to have a sponsor that cannot physically attend the Sacrament of Confirmation? You can use a PROXY. Please contact the Preparation Team to discuss this. Your sponsor still needs to fill out the form though, so go ahead and send it!
