Player's Form

Please complete all required fields in the Player Registration Sheet below.

To ensure a smooth verification process, kindly follow these steps:

1. Enter your Team Name, Manager Name, and Coach Name in the fields provided at the top of the sheet.

2. Fill in the details for all registered players (up to 12 players):

• Full Name

• NRIC / Passport Number

• Jersey Number

• Eligibility Category

3. Ensure that all information provided is accurate and complete.

4. Teams may only register one (1) male seeded player.

Once the form is completed, your submission will be reviewed by the Committee.
