How to File for a PSA in California

The CDE accepts PSA files beginning August 1st, even though the permitted timeframe to file a PSA is
October 1 through Oct 15. They must be filed each year.
How to answer each question:
Did you file an affidavit last year (Private School Affidavit 2024-25 )? You’ll want to answer “Yes”, if you did, and “No” if you did not.
Fill out our school’s information.
1. Give your school a name.
2. If you have less than 6 children, you do not need a CDS code.
3. Choose the county that you live in.
4. Choose the school district that you live in.
5. Mark the type of school that it is.
6. Mark the type of program that it is.
7. Choose whether or not you are using an IEP.
8. Mark “yes” for offering a High School diploma.
9. Choose which grades you are offering.
Next, you’ll fill in your physical and mailing address.
Then, you’ll fill out your phone number and e-mail..
In step 4 of 11, you’ll need to let them know (if you’ve already filed before) whether or not your school name has changed, and if the school district has changed.
On step 5, put how old your youngest child is, and how old your oldest child is.
Plug in your site administrator info.
Step 7: Put your name and information down as the person holding on to the records.
Under Tax Status mark “none of the above”
Step 9: Read the acknowledgement
Sign the document
Review and submit
