Policy, "No Show"

Attention New and Established Patients

Please be aware that Dr. Richard N. DeVita maintains a policy regarding missed appointments and cancellations. A fee of $25.00 will be charged in the event of a "NO-SHOW" or if an appointment is canceled within 24 hours of the scheduled time. This fee will be your responsibility and will not be billed to or covered by insurance carriers.

To schedule any future appointments with Dr. Richard N. DeVita, this fee must be paid in advance.

Additionally, please note that habitual offenders of this policy may face termination of the Doctor-Patient relationship at Dr. Richard N. DeVita's discretion.
