To ensure a smooth and efficient appointment process, kindly make sure to have the following documents and information with you when you arrive:

• Insurance Card (Dental)

• Driver's License

• Specialty Care Referral: If you are covered under Delta Care USA or Cigna HMO, please bring the necessary referral documentation. (Refer to the "What to Bring" page for details.)

• Referral Slip from Your General Dentist

• Pharmacy Information: Include the pharmacy's name, address, and phone number.

• List of Medications
