(This application is for individuals/families/siblings to apply for funding for needed item (ie: specialized items, bath chair, ramp, bed, bike, items for vision impairment, camp, sibling camp, etc. These are only examples, these items are NOT limited to these alone).

Application for Scholarship
(This application is for individuals with special needs OR individuals with family members with special needs to apply for assistance to help attend a transition school, trade school, or college. Please read requirements for scholarship).
