If you don't have a card then please try to bring information such as your member ID and group number. Sensitive information like SSN might be needed to link to insurance.

Our exams generally last an hour. Please plan accordingly. Timing can vary based on what type of testing is needed.

We try to limit our dilations, BUT it is always best to prepare to be dilated. We recommend you bring sunglasses with you and a driver if you feel uncomfortable about driving while dilated.
If you are unable to be dilated let the doctor know. He will discuss options with you.

We like to keep you with the same doctor. This allows one doctor to be familiar with your history. If you want to switch to another doctor at our practice we have to confirm with both doctors that they are ok with the switch.
