There’s a lot of helpful information there about living kidney donation, the evaluation process, and common questions. If you’re curious about how donations work, it’s a great place to start!
When you’re ready to begin the screening process, scroll down the page until you see the green button that starts the donor questionnaire. It will have you create a fast and free account first and then take you to the 5-10 minute questionnaire.

Why it’s set up this way:
This questionnaire is designed for donors to introduce themselves to the transplant team, so the “patient” field refers to you.
Confidentiality
Your questionnaire goes to the transplant team. Ryan won’t know you applied unless you choose to tell us.
