Email the chairperson/undergraduate advisor of the corresponding department and ask them to sign off on the equivalencies for the courses that you will be taking.
Be sure to include course descriptions (and syllabi, if available).
If a course does not have an exact QC equivalence, it will be granted 499 credits.
If a course is less than 3 credits, it will be granted 499 credits.
If a course is more than 3 credits, it can be split up into a 3-credit QC equivalence and the remaining credits of 499.
Replace everything in [brackets], and delete the brackets
Remember to attach the Course Approval Form to your email, and make sure to complete the top of the form, and the left side of the table.

Dear Professor [Last Name],
I hope that you are doing well.
I am writing to you because I will be studying abroad in [country] next semester, and I am planning on taking [#] [dept. name] class[es] there.
Could you please take a look at the Course Approval Form attached and then write and sign off on the corresponding Queens College equivalency? If there is not an exact QC equivalency, feel free to grant me [dept. name] 499 credits [ex: HIST 499 credits].
