CNA Information Session - Suny Attain Form -Required Documents

🛑 Before your attend information session you will need to Schedule English Test Appointment (click to schedule a test date)

Step 1️⃣ Pre-CNA Information Session

Prospective CNA Students attend Pre-CNA Mandatory Information Session on [TBA NEXT FALL SESSION]

CUNY On the Concourse, 2501 Grand Concourse, 3rd Floor.

Step 2️⃣ Complete Suny Attain Form

ATTAIN Enrollment Application

If you require assistance with the online form, we will be available to help you complete the ATTAIN form during the orientation session.

Step 3️⃣ Required Documents Submission

Please bring all your required documentation to the orientation. If you are missing any items, you may submit them via online upload to Dropbox [insert doc] or deliver them in person to the CUNY ON THE CONCOURSE (COTC) office, located at 2501 Grand Concourse, Monday through Thursday, between the hours of 10:00 AM and 4:00 PM.

1. New York State ID (Driver’s License or Non-Driver ID)

2. Unexpired: Passport OR Green Card

3. Proof of Address (e.g. letter addressed to you, lease, bill)

4. Social Security Number (or proof of application process)

5. Proof of Income (only need one):

Federal Tax Return (Form 1040) 2023 or 2024.
If your income is $0, complete the letter of support download sign it with a Notary Public, for free at your bank or UPS. **Please note: DO NOT sign the form before you go to the notary.
Shelter Letter (if you have one)
Budget Letter (for HRA recipients)

6. If you are an Asylum Seeker who crossed the border — any documents you received at the border (example: I-94 form)
