Clearance Packet

Student Athletes should:

Step 1:  Get a physical (these are good for one calendar year)

Step 2:  Fill out:  https://pvhs.pvpusd.net/apps/pages/athletics-clearance-process

Step 3:  There should be one Summary.  Take that and a current physical form to the Athletic Office.  

Please fill out all the appropriate paperwork in the clearance packet allowing parents to drive their children to meets and for athletes to drive themselves when allowed.
