PulseForm is a training and simulation application designed to support tactical medical, EMS, and instructional environments.

Information We Collect

PulseForm does not collect personal information for general app use.

However, if a user submits a request for access to SG Tracker instructor or small group leader features, PulseForm may collect limited information provided by the user for that request, including:

• Name

• Phone number or email address

How We Use Information

Information submitted through an SG Tracker access request is used only to review, approve, deny, or manage access to instructor or small group leader functions.

Data Sharing

PulseForm does not sell personal information. Information submitted for SG Tracker access requests is not shared except as necessary to manage access or as otherwise required by law.

Data Retention

Information submitted for SG Tracker access requests is retained only as long as reasonably necessary to review and manage access.

General App Use

Outside of SG Tracker access requests, PulseForm does not collect personal information for analytics, advertising, or marketing purposes.

Data Security

Reasonable measures are used to help protect information handled through the app.

Children’s Privacy

PulseForm is not directed to children under the age of 13.

Changes to This Privacy Policy

This Privacy Policy may be updated from time to time. Any updates will be posted at this same location with a revised effective date.
