This 1-year longitudinal experience includes a 2-week continuous portion and 6 longitudinal experiences consisting of 3, ½ day experiences that occur on an every 8-week schedule. These longitudinal components will occur during your weeks on continuity clinic at times when you are not scheduled to be seeing continuity patients.

Feedback and Evaluations:
The ACGME requires that all longitudinal rotations provide resident evaluations at least 4x/year. This rotation will provide the following opportunities for formal feedback in addition to any in person feedback provided by preceptors at a variety of in-person experiences:
1) A self-evaluation on personal goals during the 1st longitudinal week.
2) A self-evaluation on progress towards achieving personal goals during the 3rd longitudinal week.
3) Faculty competency-based (ABP) and milestone-linked (ACGME) formal evaluation to be completed after the 2-week continuous portion of the curriculum (time will vary depending on resident schedules).
4) A self-evaluation on progress towards achieving personal goals during the 5th longitudinal week.
*Note: It is uncommon for residents to receive formal feedback from community-based experiences. However, residents are encouraged to express learning goals for each experience and solicit real-time feedback whenever possible.
