If you have not received or have misplaced the registration renewal notice, you can contact us to request your PIN.

When e-mailing, please be sure to include your name, profession, license number, date of birth and the last 4 digits of your social security number.

This service is available for licensees in all professions who are in the final 5 months of their current registration period or no more than 4 months past the expiration of their last valid registration period. Your New York State license is good for life, however, you MUST register with us periodically if you wish to practice your profession in New York State. Upon licensure, you received a registration certificate valid for a period of three years (two years for physicians). We will mail you an application to renew your registration to practice approximately four months before your registration expires. It is your responsibility to notify this office of any change of address.

If you are not practicing your profession or using your title in New York, you may place your registration in inactive status free of charge. To do so, you must advise us of your decision to register inactive. You can do this online following the steps below or by answering "no" to question 1 on your paper renewal registration form, signing and returning the form with no fee. If you do not inform us of such a decision, you are subject to the customary registration fee for your profession. In the event that you decide to resume practice after an inactive period, you must reregister your license and meet any post-licensure requirements imposed on your profession.

This asynchronous, online, self-paced curriculum has been designed with the necessary tools, equipment and subject-matter expertise to aid you through the certificate process. It is recommended that you prepare to reserve two to three hours per lesson so you can follow along in the included study guides.

CAEP is completed online through our state-of-the-art learning management system. Each lesson has a study guide, and at the conclusion of all twenty lessons there will be an online assessment that is must be passed. When all twenty lessons are completed, and the final assessment is passed, you will have successfully demonstrated mastery, earning your CAEP certificate.

As soon as your registration fee is paid, you will have one year from that date to complete your certificate program. You will be given credentials that allow access to your learning platform.

Graduates of ABET-accredited programs who work in applied and natural science, computing, engineering and engineering technology can seek professional recognition by enhancing their credentials through licensure, registration and certification programs where appropriate. Graduation from an ABET-accredited program is increasingly a required minimum credential for such professional recognition.

In the United States, for example, where professional licensure for the engineering and surveying professions is regulated at the state level, graduation from an ABET-accredited program is almost universally required to validate the educational experience of applicants. In states where non-ABET graduates are permitted to be licensed, an additional four to eight years of work experience may be required.

Many jurisdictions require graduation from an ABET-accredited program as a minimum qualification for registration to practice because it signifies preparation for entry into the profession. For example, United States Patent and Trademark Office (USPTO) requires applicants in computing to have graduated from an ABET-accredited program before they are eligible to sit for the Examination for Registration to Practice in Patent Cases.

To promote the global career mobility of technical professionals, we participate in a number of Mutual Recognition Agreements (MRA). Graduates of programs accredited by an agency that is a signatory to these MRAs may be recognized for the purposes of licensure, registration or certification.

National Council of Examiners for Engineering and Surveying

NCEES is a nonprofit organization dedicated to advancing professional licensure for engineers and surveyors. It develops, administers, and scores the examinations used for engineering and surveying licensure in the United States.

SME, a nonprofit association of professionals, educators and students committed to promoting and supporting the manufacturing industry, offers several certification programs for manufacturing professionals.

The purpose of ARRT certification and registration is to recognize individuals qualified to perform a specific role. For example, the purpose of our Radiography certification and registration is to recognize individuals who are qualified to perform the role of a radiographer. Certification and registration requires satisfaction of certain professional standards in medical imaging, interventional procedures, or radiation therapy.

An Initial Two-Year teacher certificate will be valid for two years and may be extended once at no charge for one-year so long as the extension requirements have been met before the expiration date of the Initial Two-Year teacher certificate.

An applicant who has satisfied the requirments for Initial Two-Year teacher certification as asll as the Alaska studies and Alaska multicultural coursework requirements at the time of application, will be issued an Initial Three-Year teacher certificate.

The Initial Out-of-State teacher certificate is for educators who are currently certified in another state to teach, and who would like to begin teaching in Alaska. To qualify for an Initial Out-of-State teacher certificate an applicant must meet the following requirements:

An applicant who has held a Reemployment teacher certificate must apply for either a Professional Teacher Certificate or a Second Initial Certificate if they do not meet the Professional Teacher Certificate Requirements.

An Initial/Reemployment teacher certificate will be valid for one year and is not renewable. The Initial/Reemployment teacher certificate cannot be reissued and is not eligible for an extension.

Evergreen does not require a full application for most academic certificates. Please declare your intent to earn a certificate on the enrollment form. Enrollment for certificates starting in Fall 2023 will begin May 1, 2023.

These programs may be offered in partnership through outside agencies and may have additional application/enrollment and registration requirements. Review the certificate page for specific enrollment information and contact certificates@evergreen.edu with questions.

Students may register for academic offerings that are a component of a certificate without enrolling in the certificate. After registering for the first offering, students enrolled in a certificate with a set sequence of offerings have priority registration for associated academic offerings over students not enrolled in the certificate. Students must be enrolled in the certificate and registered in the first course of the certificate sequence or a core course to have priority registration for subsequent certificate offerings.

Students enrolled in a certificate program are expected to complete the requirements of the certificate as designed in sequence in a single academic year. Students may opt to delay completing all requirements for a certificate program for one year, provided the certificate program continues to be offered. Students delaying more than a year without completing the certificate will no longer be enrolled in the certificate. Students wishing to pursue the certificate again must re-enroll and pay an additional certificate enrollment fee.

Students must be enrolled in the certificate program to earn a certificate credential. Students are encouraged to enroll in the certificate prior to their first certificate class. However, students are allowed to enroll any time up until the 2nd Friday of the quarter of their final certificate course. In exceptional circumstances a student may appeal to the dean for late certificate registration.

Students enrolling in a certificate program are required to pay an enrollment fee. The enrollment fee is reduced by 50% for students currently seeking an Evergreen degree. The fee covers enrollment and delivery of the certificate credential upon completion. It does not cover tuition and other fees associated with registration in the academic offerings required for the certificate.

Academic certificate offerings will normally be provided as state-supported, with tuition charged according to the regular tuition table. However, academic certificate offerings may also be provided as components of a self-sustaining unit such as Summer School, for which the tuition may vary. 2351a5e196
