Our Goals

• Improve access to family physicians and nurse practitioners for unattached patients.

• Support patients in family doctor access and identifying clinics accepting new patients.

• Reduce barriers related to language, health literacy, and system complexity.

• Promote interprofessional collaboration between nursing, medical students and healthcare professionals.

• Strengthen community health by connecting people with preventive and ongoing care.

What We Do

Primary Care Navigation Sessions  

• Students meet with patients one‑on‑one or in small groups to help them:

• • Sign up with Primary Care Clinics

  • Identify nearby clinics and community health centres

  • Understand walk‑in, urgent care, and virtual care options

  • Support residents regardless of races and status towards Primary Care
