Plungers under 18 years of age must print and fill out this waiver, have a parent or guardian sign it, and return it to the SOMT state office no later than 3 days prior to the plunge. Adult plungers signed the waiver digitally when they registered.

If you are a team leader with a team of plungers under the age of 18, it is recommended that you print, distribute, and collect all waivers and send them in as a group.
